Introduction to Toothrot Tilly
Toothrot Tilly is a casino slot from Pineapple Play for those with a bit of a sweet tooth. You might not be familiar with the studio, as this is Pineapple Play’s third-ever release, and it’s a name that’s quite new to the scene, debuting in late 2025 with the release of Coconut Chaos, and then followed up with Midnight Mirage. While these first two releases weren’t very special, we want to say that, this time around, they do deliver on a pretty interesting twist. Candy themes have been one of the simplest, and arguably one of the most basic themes in the industry for quite some time, only behind the old classics such as Fruit slots. But Pineapple Play takes this idea and makes it a bit stranger with an over-the-top battle between the titular sugar-loving Tilly and the Tooth Fairy herself. The gameplay loop then follows us cashing in on Wrapped Candy, Chocolates, and Rotten Wilds for Prizes and Multipliers on Cascade reels, until we hit the Free Spins, where the two characters start fighting in a Beam Battle, awarding us with tons of small bonuses as the tide of battle changes. Tag along as we check out what this new up-and-coming studio does with a typical candy-theme, delivering on good spins and a max win of 10,000X the bet.

Theme, Gameplay, & Payouts
Toothrot Tilly is a video slot from Pineapple Play with 5 Reels, a 3-4-5-4-3 Row Setup, and uses Scatter Pay Mechanics, needing a minimum of 5+ of the same symbols anywhere on the reels to form a win. You can choose between making a Min.bet of 0.1 and a Max.bet of 100. Toothrot Tilly comes with a slightly higher-than-average RTP of 96.34%, medium volatility, and a solid max win of 10,000X the bet.
Set inside a candy store after dark, Toothrot Tilly finds her way inside to gobble up just about anything she can set her eyes on, with the Tooth Fairy showing up to stop her. The game definitely doesn’t take itself seriously, as can be seen when the bonus game triggers and the two characters start a magical battle. You can play the game on most notepads, mobiles, and desktops.
The lower-paying symbols are various gummy teddy bears in various colors, including green, orange, purple, pink, and yellow. Creating a win with 13+ of the same symbols will pay out 5X to 20X the bet. The higher-paying symbols are various sweets and characters, including green macarons, blue candy-floss, purple cake, the tooth fairy, and Tilly. Landing a win with 13+ of the same symbols pays out 44X to 120X the bet. Then we have quite a few Special Symbols, ranging from Wilds, Rotten Wilds, Scatters, Wrapped Candy, and Wrapped Chocolate Symbols, and you can learn more about these in the Feature section.
Special Features & Mechanics
What is the max win in the slot Toothrot Tilly?
You can win up to 10,000X the bet.
What features are available in the slot Toothrot Tilly?
The features are Wild Symbols, Cascades, Rotten Wild Symbols, Wrapped Symbols, Molar Reel Bonuses, Cavity Clash, and Feature Buy.

Wild Symbols
Wild Symbols help form winning combinations by being able to substitute for all paying symbols, and don’t substitute for any Special Symbols such as Scatters, Wrapped, or Rotten Wild Symbols. Only 1 Wild Symbol may be visible on the grid at any one time, and it is removed after participating in a win.
Cascades
After landing a Scatter Pay winning combination, the winning symbols are removed from the playing grid, allowing new symbols to fall down and potentially create new wins. During the Cascades, all wins will be added to a Total Win counter, which will payout at the end of a spin, and the Cascades will continue until no more wins can be created.
Rotten Wild Symbols
The Rotten Wild Symbols can only appear after landing at least three winning combinations during a single spin or Cascade. The Special Symbol will act as a regular Wild, substituting for paying symbols, and also offers a random Multiplier value of 2X-40X.
Wrapped Symbols
There are two types of Wrapped Symbols, Wrapped Candy, which comes with random cash values ranging from 2X to 250X, and Wrapped Chocolate, which contains Multipliers of 2X to 40X. These can only be unwrapped and rewarded through the Molar Reel bonuses Sticky Fingers or Fairy Flicks, and will instantly be added to the Total Win Bar after being unwrapped.
Molar Reel Bonuses
The Molar Reel is a special box outside of the playing grid that spins alongside the reels, and can randomly award one of three bonuses to be triggered for that spin. They are as follows:
Sticky Fingers = When triggered, all visible Wrapped Candy and Wrapped Chocolate are removed from the grid, unwrapping and awarding their prizes, as well as causing a cascade.
Fairy Flick = When triggered, the Fairy Flick will choose one of two sections, removing all Symbols except Scatter, Wild, and Rotten Wild Symbols from the section, causing a cascade.
Sugar Swirl = When triggered, all low-paying symbols visible on the grid are converted into the highest paying low-paying symbol visible on the grid.
Cavity Clash
By landing 3 Scatter Symbols, you will be awarded the Cavity Clash Free Spins feature with 10 spins. During the bonus game, a Beam Battle between Tilly and the Tooth Fairy will be present, and by landing golden-framed character symbols, the beam will move for the respective character. A Next Spin Bar will also appear, showing what guaranteed bonus effects will happen on the next spin, depending on which position the Beam is on, and they are as follows.
1 Space Towards Tilly = Guarantees one or more Wrapped Candy Symbols to land.
2 Space Towards Tilly = Removes Green, Orange, and Purple low-paying symbols from the reels.
3 Space Towards Tilly = Guarantees one or more Wrapped Chocolate Symbols to land, and one of the 3 Molar Reel bonuses to trigger.
4 Space Towards Tilly = Removes all low-paying symbols for the next spin, and Fairy Flick or Sticky Fingers is guaranteed to activate.
1 Space Towards The Tooth Fairy = Guarantees one or more Wrapped Chocolate Symbols to land.
2 Space Towards The Tooth Fairy = Guarantees to trigger the Fairy Flick bonus.
3 Space Towards The Tooth Fairy = Guarantees a Wild Symbol will land.
4 Space Towards The Tooth Fairy = Guarantees a Rotten Wild Symbol will land.
Reaching 4 spaces toward either Tilly or the Tooth Fairy for the first time will award +2 extra spins, giving you the possibility of getting a maximum of +4 extra spins during a single bonus game.

Feature Buy
Through the Feature Buy, you can instantly trigger the various features and bonuses for a price based on the current bet.
3X = Triggers the BonusHunt FeatureSpins, making each spin 5 times more likely to trigger the Cavity Clash bonus game.
50X = Triggers the Tilly’s Temptation: FeatureSpins, making one of the three Molar Reel Bonuses guaranteed to trigger on each spin.
100X = Awards the Cavity Clash bonus game.
Final Thoughts on Toothrot Tilly
While Pineapple Play’s first two releases weren’t anything that really left a lasting impression on us, Toothrot Tilly does a better job at showing off their strengths, and it makes it even more apparent that the studio is partnered with Hacksaw Gaming through their Open RGS. We really enjoyed the silly, over-the-top theme and setting, however, it’s not like that’s everything, and it can carry this game to greatness. The studio delivered something different, both thematically and gameplay-wise, as they try to push familiar features slightly, which has its pros and cons.
The gameplay loop, at a glance, seems like something we’ve seen many times before, being boiled down to a Cascade game filled with Cash Prizes and Multipliers, however, as we said before, Pineapple Play did try something different through its Wrapped Symbols and the Molar Reel bonuses. Through the use of the Total Win Bar, any Cascade can quickly stack up its wins, even in the base game, as Wrapped Symbols can slowly fall in over the course of a spin, making you wait for the Molar Reel bonus to activate, which means some pretty anticipating and engaging spins as you wait to unwrap your prizes. However, coupled with the many bonuses in the Cavity Clash bonus game and keeping track of the Rotten Wilds counter, we do think the game can feel a bit mechanically overwhelming. While we personally don’t have an issue with it, as we enjoy all types of slot games, we can see it not jiving with all sorts of players, as it can take some time to really wrap your head around it. All in all, Toothrot Tilly is a fun, new release from an upcoming studio that tries to deliver on something new, and does so in a pretty neat package, but with its ups and downs that can easily split an audience.
Toothrot Tilly is Pineapple Play’s third-ever release, delivering on a strange twist on a common theme and fun spins through Wrapped Prizes and tons of bonuses.
